Aim
The aim of this assignment is to help you understand medication safety in clinical practice, the avoidance of error and the importance of patient education.
Assessment Guidelines
A patient in your ward has been prescribed a new medicine during this admission and is due for discharge home in two days. Choose a typical patient and medication to create a framework for your case study, and analyse the important steps related to medicines management and safety in preparation for discharge using the sections below.
Section 1 (35 marks)
Analyse the role of the registered nurse in medication management and safety while caring for the patient in hospital. Make specific reference to safety and the avoidance of medication incidents, errors and near misses. Include in your analysis allergies, drug-drug interactions and drug-food interactions.
Section 2 (20 marks)
Give a brief overview of how to investigate and document an actual medication error in a typical hospital ward location. Include the role of the various clinicians who would most likely be involved in this investigation.
Section 3 (40 marks)
Discuss the role of the nurse in patient education for medicines management, with particular reference to preparing the patient for discharge home. Create a patient education plan for the patient, considering all the important safety information and who the patient should or could contact for further advice after discharge.
