The Learner is required to complete a journal of learning as a reflection of their experiences while working with a person/people with a disability.
This is to serve as a demonstration of their ability to reflect on their role in supporting a person with a disability and self-assessment of same.
Learners are required to submit five separate journal entries using the specified criteria as outlined in this brief/ within the guidelines below.
Each entry should be based on a different task or activity carried out with the person/ people that the learner is supporting.
Presentation of Assignment
- Cover Page (detailing your name, student number, module title, module code, title of the assignment, name of the assignment and a contents page)
- Content: Five separate journal entries, each based on a different task or activity.
- Reference List: Harvard style referencing
- Word Count: 3000-3500 written words (approximately 600-700 words per entry)
- Additional Notes for Learners:
- Clarity and Professionalism: Ensure your journal entries are well-structured, clear and professional in tone. Use examples and specific details to demonstrate your reflection and learning.
- Evidence of Learning: Each entry should reflect critical thinking and self-awareness. Discuss both positive outcomes and challenges and be honest in identifying areas for growth.

Guidelines
You are required to complete five separate journal entries, each based on a different task or activity.
Each task/ activity should be examined as follows:
1. Activity Overview (2 Marks)
- Describe the task or activity you carried out, including when and where it happened.
- List the skills and personal qualities you needed to support the person effectively. Mention any skills you used or developed during the activity.
- Identify any laws or guidelines that protect the rights of the person you were supporting.
2. Supporting the Person (2 Marks)
- Outline how you supported the person in this task
- Mention any guidelines or rules you followed, such as those from your workplace, legal requirements or best practices for supporting people with disabilities.
(This can include specific steps you took to ensure the person’s safety, respect and dignity.)
3. Principles Used (2 Marks)
- Explain how you applied these principles:
- Individuality (treating the person as unique)
- Personal Autonomy (promoting the person’s right to make their own choices)
- Effective Participation (encouraging the person’s active involvement)
4. Guidelines & Lessons Learned (2 Marks)
- Mention any ‘best practice’ guidelines you followed and how they helped ensure quality and safety in your work.
- Share what you learned from this activity, including key insights and how these may influence your future actions.
(e.g. What aspects of the activity/ task did you enjoy? What did you find challenging?/ If you were to repeat it what would you do differently?)
5. Relationship Management (2 Marks)
- List any other people involved or may have an interest in the task (such as family members, teachers or any other MDT members).
- Discuss how you worked with these individuals and what you did to build effective and supportive relationships.
