NCAD, Skills logs and Written Reflections
Do not include any data which may identify a patient or healthcare provider in any part of your assessments.
This assessment supports you to achieve the following module learning outcome:
1.Demonstrate competence in evidence-based skills associated with creating and maintaining a safe environment for patients under supervision of a registered nurse/preceptor.
2.Participate and reflect on assisting patients with activities of living (AL) under supervision as part of the nursing team.
3.Identify, implement and reflect on nursing interventions appropriate to scope of nursing practice for stage 1 students
4.Relate theory, participate and reflect on patient care under supervision in interventions relating to vital signs and the promotion of skin integrity as appropriate to a Stage 1 nursing student.
5.Articulate and demonstrate competence a range of techniques in communicating with colleagues, patients and families in the healthcare context as appropriate to a Stage 1 nursing student.
6.Distinguish situations beyond personal competence and report to supervising nurse and to other healthcare professionals.

Aim of assessment
The aim of this formative assessment is to support you in the creation of your NCAD and NCAD support clinical competencies and written reflections.

Minimum requirements for collections
Part 1: National Competence Assessment Document NCAD
Developing NCAD collection — demonstrates the development of competence through the practice of key performance indicators using the NCAD under supervision in clinical learning environments:
In this part, you must complete the NCAD record using the appropriate NMBI template. These NCAD’s must be submitted in person on completion of each placement and recorded as submitted. Total of 3 NCAD’s in this stage.
Part 2: Skills Logs
Developing competence collection — demonstrates the development of competence through the practice of key skills in simulated and clinical learning environments:
In this part, you must complete the clinical skills record using the appropriate template – paper version for ease of use in clinical placement and electronic in your professional portfolio.
The paper versions must be scanned and added to the clinical skills area of your portfolio as supporting evidence. The use of the Educational Template is advised.
The student is responsible to ensure that the image quality of the scanned paper version can be clearly read.

Part 3: Written reflections
Developing knowledge using reflection as a collection – demonstrates the development of clinical knowledge and reflective skills during specialist clinical learning environments:
In this part you must create and complete a collection containing 2 pieces of reflective writing for every 6 weeks of clinical placement using the appropriate template. The use of the Gibbs template is a requirement of the NMBI here.
These reflections must be submitted to MyHELMS assessment portal at the end of each 6-week placement. A total of 6 reflections are required in this stage.
Note: Failure to complete any of these parts will result in an incomplete module and may attract a fail grade. This may impact progression to the next stage of the programme.
