JR is a 17-year-old teenager with a mild intellectual disability who lives at home with his parents and two older brothers and maintains a good relationship with them. Recently, he has exhibited destructive behaviours, expressing frustration about his future. He has shared his feelings of isolation with both his parents and the community nursing team, particularly as he watches his friends explore postsecondary school opportunities, a future he does not see for himself turning 18 in 5 months’ time. JR feels he is perceived differently by others and is not seen as having the same value in society. This growing sense of uncertainty has impacted his emotional well-being, and his family has reported changes in his behaviour, including JR causing damage to property and his surroundings.

JR also has epilepsy, experiencing at least one tonic-clonic seizure per month, requiring rescue medication for clinical management. At times, this health condition has influenced his social engagement and access to opportunities, further impacting his perception of his future. JR can communicate verbally, but only people who know him well can understand him clearly, which is another source of JR’s frustration.

As an RNID in the Children’s Disability Network Team, you are required to review

JR’s needs, as he will soon be transitioning out of children’s services. Using a rightsbased approach, discuss a person-centred strategy to evaluate JR’s needs in two of the following areas:

  • Transition
  • Psychosocial needs

Assessment Guidelines

Introduction (15 marks)
