Part I: Description of Entire Current System
You are required to describe an existing manual or electronic system or hybrid (i.e. an aspect(s) is manual with another/others electronic) with which you are familiar. This system should be in use in a healthcare context to support healthcare administration and/or healthcare delivery. Your description should at least identify the system stakeholders, the functionality afforded to each of these, and details of interactions between this system and other systems internal and/or external to the organisation.
Part II: Detailed Description and Assessment of Current System/Sub-System
For the remainder of this assignment, you are either going to deal with this entire current system or a sub-system of this current system. If the system you described in Part I is very comprehensive, for the purpose of the remainder of this assignment, you should concentrate on an appropriate aspect of this system i.e. a sub-system. At the very outset in this section of your submission, please state whether it is the entire system or a particular part(s) that will be the focus of the remainder of your submission (explicitly state which aspect(s)).
Also, you should provide a detailed description of the current system or sub-system which should include details of the flows of data/information and the entities (people, organisations, other systems and so forth) involved as sources (originators) and/or sinks (recipients) of this data/information. You may use diagrams to support your description, but you are not expected to draw a Context Diagram (CD) and/or a Top Level Data Flow Diagram (TL-DFD) at this stage. You should structure your description in an appropriate manner, paying particular reference to, how data/information is collected, processed, distributed, reported on, stored, its retention and disposal, the stakeholders involved, any other system(s) internal and/or external involved, legislation/regulation/rules/processes/procedures to be complied with and so forth.
In addition, you are to provide an assessment of the current data/information governance in place at your healthcare organisation (as appropriate), paying particular reference to, the system/sub-system you have chosen. You should identify, justify, and apply appropriate assessment criteria as well as give an overall assessment of the data/information governance. In addition, your assessment should provide details of any limitations and associated
risks (e.g. technological, human, usability, …), identifying in your view the levels of such risks (very high, high, moderate, low, very low).
Part III: Description of Proposed System/Sub-System
You are required to provide a description of your proposed system (sub-system) which overcomes the limitations, mitigates against the risks including any improvements concerning information governance (identified in Part II).
Your description should include:
(a) details of the flows of information and the entities (people, organisations, other systems and so forth) as sources
(originators) and/or sinks (recipients) of this information.
(b) draw a CD and a TL-DFD to model your proposed system. These diagrams may be drawn using a tool or freehand.
For each
- external entity in your TL-DFD, complete the External Entity Template (cf. Table 1)
- data flow in your TL-DFD, complete the Data Flow Template (cf. Table 2)
- process in your TL-DFD, complete the Process Template (cf. Table 3).
(c) using the data stores from your TL-DFD, identify the set of relations/tables required to support the proposed system. For the relations identified, using a database management tool, show the resulting tables and the relationships between them.
Your final set of relations should include those required to support improvements in information governance. In your submission, you should include a screen capture(s) showing these relationships. For each table you include in your solution, specify for each attribute a brief description, its data type, length, any constraints and validation rules (cf. Table 4).
(d) You should modify the captions on the tables mentioned in b) and c) accordingly.
Part IV: Re-Visiting System Assessment
In Part II (b) you identified, justified and applied data/information governance assessment criteria to the existing system (sub-system). You should now reconsider these criteria, revise accordingly and apply to your proposed solution. In addition, you should give an overall assessment of the data/information governance of your proposed solution.
