LO1- Appraise how lifestyle choice and wider determinants of health impacts upon health and wellbeing.

LO2- Demonstrate a range of theoretical concepts and frameworks used to promote and protect health and prevent ill health.

LO3- Identify inter-agency involvement in shared decision making in promoting and protecting health across a range of settings.

The Summative Assignment Requires:

An Infographic, structured supplementary information and a Harvard style reference list to be submitted as one piece of work on either a Word document or Power-Point

1. Infographic:    LO1 and LO3 need to be evident in this section  

Demonstrate your learning across all learning outcomes by creating an Infographic for a chosen health topic accompanied by oral or written supplementary information.

It would be beneficial for your Infographic to include:

•      A concise and creative title and strapline that draws the attention of your target audience.

•      Evidence of statistical data to support your evidence-based information related to your health topic, using; charts/ graphs/ tables.

•      Relevant and appropriate (copywrite free) images to support your evidencebased information/ findings.

•      Present information on how lifestyle choices may impact upon health and wellbeing related to your chosen health topic that support positive behavioural change.

•      Demonstrate the ability to link wider determinants of health that have an impact on your chosen health need/topic.

•      Identify inter-agency involvement in the promotion and protection of health relevant to your health topic.  

•      Adherence to health literacy guidelines is required.

1. Structured Supplementary Information (minimum suggested wordage 200-300 words): 

LO2 needs to be evident in this section to support the theoretical approach/es that you have used in your infographic.  There should be new information to support LO1 and LO3  

The accompanying supplementary information (oral or written) is required to elaborate on points of relevance raised in your Infographic.

You must structure your supplementary information as below using these headings:

Background

To provide a short summary of the reason for selecting your health topic (i.e. what are the evidence-based drivers for looking at this?)

Aims/Purpose

What are the expected outcomes from creating your infographic? What do you want to achieve? (Raising awareness, education, etc.)

Discussion

Supportive discussion of key theories and frameworks that are used in the development of your infographic (ie. Levels of health promotion, health promotion approaches, behaviour change models / theory etc.) Enhancing information regarding inter-agency working.

Conclusion

Present the key messages that have emerged from the evidence that you have appraised.

Please note:

Your 500 words (+10%) includes text in your Infographic and your Supplementary Information. The wordage includes headings and intext citations, and therefore your information needs to be clear and concise.

2. Reference List

A single reference list is required that covers citations for both the Infographic and the Supplementary Information for this submission. You must use Harvard Referencing System for your reference list.

Some Key Points to Consider

Have you:

✓  Used language that is clear and concise and adhered to the word limit for this assignment?

✓  Checked all references are included and that they are referenced correctly?

✓  Checked spelling, punctuation and grammar (unless you have been assessed for considerate marking)

✓  Used understandable and accessible language (Health literacy guidance)?

✓  Considered and sensitive language appropriate to the population group discussed (i.e. transgender, ethnicity, disability etc)
