In this assignment the learner should:

·        Identify the hygiene and health care needs of the identified client

·         Discuss the care needs in terms of physical, emotional, social, psychological and spiritual impact on the person

·         Discuss how these needs are being met

·         Discuss procedures relating to the safety and security of the clients in terms of meeting their needs

·         Discuss the importance of safe and hygienic work practices

·         Identify the key points that will contribute to an enhanced quality of life for the client

·         Identify reporting procedures for changes in client’s wellbeing

·         Identify relevant records that must be maintained on clients

Summarise the discussion, draw conclusions and make relevant recommendations

Assessment Criteria/Marking Scheme:

The learner should be required to identify and profile a distinct client with a range of care needs. It should focus on the needs of the client such as emotional, social and personal development needs and client participation in activities.

You will use Mary O’ Connor as the client, the following is her current situation:

Mary O’Connor is a 78-year-old woman living independently at home with daily support from a home care assistant and weekly visits from a PHN. Those providing care to Mary, use a communication log book to document care given. She was diagnosed with

Alzheimer’s disease (a form of dementia) three years ago. Mary is a retired schoolteacher and widow. She has two adult children who live nearby and visit regularly. Mary enjoys gardening and is deeply spiritual, regularly praying and watching mass online.

While Mary is physically well, she is experiencing increasing short-term memory loss, confusion, and occasional disorientation which contributes to her risk of falls. Mary suffers from urinary incontinence and requires assistance with this activity of daily living. She can no longer manage her medication or prepare meals safely. She sometimes forgets to eat, turn off appliances, or lock the front door.

You are the home carer assigned to Mary

  • Definition of Alzeimhers disease (1mark) and identify 4 signs and symptoms (1 mark)
  • Identify three needs (Social, physical, psychological, spiritual and emotional) that you think are a priority in this patient’s care and discuss how you can meet these needs for Mary. (5mark for the need and 1.5 marks meeting the needs = 6marks)
  • Discuss safety and security: How can we keep Mary safe in her home environment (2marks)
  • Discuss safe and hygienic work practices: Discuss two points (2marks)
  • Identify relevant records that must be maintained for Mary’s care: Discuss two points (2marks)
  • Identify reporting procedures for any changes in Mary’s well being: Discuss two points (2marks)
  • Discuss two points to enhance Mary’s quality of life: Discuss two points (2marks)
  • Conclusion and Bibliography (2 marks)
